Advanced Practice Clinician ( NP/PA) - Yakima County, WA

Position Overview

Join Optum’s HouseCalls team to deliver in-home annual clinical assessments that help identify health risks and coordinate follow-up care for patients. This role is part of Optum’s Home and Community Care division, focused on providing holistic, compassionate care to vulnerable populations.

Position Highlights

• Flexible scheduling across a 7-day workweek
• No on-call, weekends, or holidays

Primary Responsibilities

• Conduct in-home health assessments and physical exams
• Perform screenings and point-of-care testing
• Identify and document medical conditions
• Communicate findings to primary care providers
• Educate patients on disease management and medication adherence

Required:

• Active NP or PA license (unencumbered)
• National certification (ANCC, AANP, or NCCPA)
• Ability to lift 30 lbs, navigate stairs, and perform physical assessments
• Reliable transportation

Preferred:

• Experience in geriatrics, cardiology, internal medicine, or endocrinology
• Home health or house call experience
• Multilingual fluency (Spanish, Cantonese, Korean, Vietnamese, Polish, etc.)
